A few short forms are completed before your session to help me understand where you are and how to best support you. They also ensure clarity, confidentiality, and a safe, respectful space for our work together.

Intake Form: Our intake form helps me understand your current physical, emotional, and energetic state so I can tailor each session to your needs. It also ensures we create a safe, clear, and supportive space for your experience.

Reflection Form: After your session, this form gives you space to reflect on your experience, notice any shifts, and integrate what came up. It also helps me better understand your journey and support you moving forward.

Liability Waiver: This form outlines the nature of the services provided and clarifies that sessions are not a substitute for medical or psychological care. It helps ensure clear expectations and protects both you and the practitioner.
